There was a hard fork at block 10,000 from a post-tag commit.  Maybe new witnesses got voted in around 14,000 who hadn't gotten that upgrade yet and dropped us below the required participation level.

That is exactly what happened.   Bm didn't upgrade the init node.

and why have we not identified the event just after it occurred and only a couple of days later?

Because the market was thin

Found a bug
Trying to create a new account name, I got:
  Failed to broadcast the transaction:
     fee_paying_account->is_lifetime_member():

Not a bug. Only lifetime members can create accounts.

Monster you are being intentionally difficult.   Just because a merchant can choose to trust all witnesses does not mean he has to.   Once the attack happened once everyone would move to requiring more than one confirmation.  You are effectively insisting that front doors are not secure because everyone leaves them unlocked.  As soon as there is a crime spree the doors will lock up.

Any delay on incoming votes also reduces response times when it matters most.   I suspect fast responses to outside threats are more important than slowing down internal threats.

There are many dynamics of approval voting that these ideas fail to consider. 

Limiting the number you can approve only decreases security.  Agent 86 and bm fought over that a long time ago
